Scoliosis Explained: Key Facts Every Singaporean Should Know

Scoliosis, a condition where the spine curves sideways, often develops silently, making early detection challenging. While a healthy spine appears straight from the front or back, scoliosis creates an abnormal curvature that can affect posture, appearance, and in some cases, cause discomfort. Orthopaedic experts say understanding the symptoms and treatment options is crucial for both children and adults.

What Is Scoliosis?

Scoliosis affects around two to three percent of the population, with most cases being mild. The condition can be identified at any age but is most commonly diagnosed during adolescence and in older adults. There are several types of scoliosis:

  • Idiopathic scoliosis: The most common form, with no clearly identifiable cause.

  • Congenital scoliosis: Present at birth due to spinal development anomalies.

  • Neuromuscular scoliosis: Linked to conditions affecting muscles or nerves.

  • Degenerative scoliosis: Typically seen in older adults due to age-related spinal changes.

Early Signs to Watch For

Detecting scoliosis early is key, especially in children and teenagers. The condition is often painless in younger patients, but subtle physical signs can include:

  • Uneven shoulders or waistline

  • One shoulder blade that appears more prominent

  • Clothing that hangs unevenly on one side

Teenagers may hide these signs with loose clothing or long hair, which is why routine screening and early evaluation are important. Adults may experience back pain or stiffness, particularly if scoliosis has progressed over time.

What Causes Scoliosis?

While idiopathic scoliosis does not have a single cause, research points to a genetic component, often running in families. Girls are significantly more likely to develop the condition than boys, with rapid progression commonly observed during puberty.

Treatment Options

Management of scoliosis depends on the type, severity, and risk of progression. Surgery is not always necessary, and three primary strategies are available:

  1. Scoliosis-specific workouts: Exercises tailored to improve spinal alignment and support, suitable for all levels but most effective for mild cases.

  2. Bracing: Recommended for moderate curves at risk of worsening. Braces are typically worn for 13 hours a day over two to three years and can help prevent further curvature. Success depends on consistent use.

  3. Surgery: Severe scoliosis may require spinal correction surgery. Traditional spinal fusion stabilises the spine but can reduce flexibility. Minimally invasive techniques, including anterior vertebral body tethering (aVBT), offer options to guide growth while preserving mobility, making them suitable for young or active patients.

Life After Treatment

Most patients return to normal activities after recovery, with over 90 percent reporting high satisfaction. Daily routines, sports, and hobbies can usually be resumed, and women with scoliosis surgery can have normal pregnancies and deliveries.

Taking Action

Regular check-ups are essential for early detection, particularly for children and teenagers. Parents should look for asymmetry in posture, and adults experiencing back pain or noticeable spinal changes should consult a specialist. Early awareness and proper management can help maintain spinal health, improve comfort, and support an active lifestyle.
